It's that time again! The SAMHSA SOAR TA Center is collecting outcomes on SSI/SSDI applications (initial and appeal) with decisions in FY 2015 (July 1, 2014 -June 30, 2015).

If you have not submitted your outcomes we are asking for you to PLEASE send us those reports!
We are collecting outcomes from July 1, 2014 through June 30, 2015 ONLY. If you have NEVER reported to us before, then please report all the outcomes you have. Be sure to include the dates the outcomes represent. Please record your outcomes and program information on the
provided spreadsheet (link initiates download of an Excel spreadsheet) and return it no later than Friday, August 28th.
