Dear State PATH Contacts and PATH Providers:

As a reminder, the proposed PATH annual report was released for public comment in a Federal Register Notice (FRN) on May 18, 2015. The FRN provides a summary of the proposed changes to the report, and the full proposed PATH Annual Report data elements and guidance is available here.

Please note that the manual is not final and PATH providers should continue to collect data according to the current PATH Annual Report form and PATH Annual Report Provider Guide. The proposed data elements and guidance are subject to change as a result of the public review and comment period.
